OBJECT OF THE INVENTION
The present invention falls within the technical field of devices for introduction, guidance, advancement, placement or maintenance in position of hollow sheaths, catheters and probes, and refers in particular to a device that allows access, guidance and fixation. of said elements, especially designed for the case of probes used in percutaneous surgery.
BACKGROUND OF THE INVENTION
Within minimally invasive techniques, percutaneous surgery is one that is performed through minimal skin incisions, performed with needles, guides, catheters
or similar, controlled and guided by diagnostic techniques by imaging tests such as ultrasound, tomography or resonance, and presents as a fundamental advantage the fact that, by eliminating surgical aggression, the patient has a lower anesthetic requirement and recovery is faster .
In the medical field of urology there are a series of percutaneous procedures that are used either to evacuate urine from a kidney or to eliminate kidney stones that consist of the introduction, through a small incision made in the lumbar area, of a catheter to the kidney.
In the first case, the operation is known as nephrostomy and is performed due to the difficulty of the urine to be evacuated through the urethra, which can cause hydronephrosis, which may be due to the presence of kidney stones. Urine can accumulate in the kidney and cause kidney failure, urinary infections and pyelonephritis or inflammation of the kidney.
The second type of percutaneous intervention in the kidney is called nephrostolithotomy or nephrolithotomy, and it involves the removal of stones accumulated in the kidney that, due to their size, cannot be evacuated through the urinary tract.
The introduction of catheters for intervention presents a specific problem related to the movement or loss of the position that occurs when moving forward and moving inside the body, as well as possible bleeding in the area of the incision due to said movement.
DESCRIPTION OF THE INVENTION
The object of the invention consists of a device for introducing, guiding and fixing the surgical instruments used in percutaneous surgery, which is especially designed for sheaths, probes and catheters. The device comprises a plurality of elements arranged in essentially concentric layers, and is sized to be inserted over the skin tissue of the individual undergoing percutaneous intervention, at the exact point at which the incision is made.
Said elements that make up the device have a hole in its central part, intended to be arranged coincidentally with the point of incision in the skin of the individual, to allow passage through the sheath or instrument in question into the interior of the organism.
The first layer has an adhesive coating on its back, intended to come into direct contact with the skin, so that the device is fixed once it is placed in the appropriate position. Said first layer is preferably made of flexible materials that allow the device to adapt to the morphology of the area of the body where the incision is made to access the body.
A second layer made of rigid material, concentrically arranged on the first layer and with a lower surface than this, acts as a support for a hollow cannula-like entrance channel that, starting from the surface of said second layer, is projected towards the outer face Of the device. The channel is conceived as a way of introducing the sheath, and has means to guide, fix the position, and if necessary, retain, the sheath inserted in the organism, once the necessary orientation has been obtained.
The means for retaining the sheath in the channel can comprise a manually operated clamp-type perimeter retaining ring, to fix the longitudinal position, an articulated retention ball, to keep the sheath at a certain inclination, or additionally plastic villi that start from the interior perimeter of said channel.
The option is contemplated that the device is made of polymeric materials with memory, to facilitate the introduction of the sheath in its optimal position.
The device for introducing, guiding and fixing percutaneous surgical instruments thus described allows the creation of a safe and comfortable access route for both the patient and the surgeon, avoiding problems arising from unwanted movements of the inserted sheath.

To complement the description that is being made and in order to help a better understanding of the characteristics of the invention, according to a preferred example of practical implementation thereof, a set of drawings is attached as an integral part of said description. where, for illustrative and non-limiting purposes, the following has been represented:

PREFERRED EMBODIMENT OF THE INVENTION
Next, a detailed explanation of a preferred embodiment of the object of the present invention is provided with the aid of the aforementioned figures.
The device for introducing, guiding and fixing percutaneous surgical instruments described is specially designed and sized for the case of renal surgery percutaneously, in which a sheath is inserted
(1) hollow, through a skin incision made in the lumbar area of an individual, until reaching the kidney.
Said device comprises a first laminar body (2) with a flat surface and essentially circular geometry as shown in Figure 1, intended to act as a means of joining the device with the body of the individual. The first body (2) comprises a first outer face, coated with any one of the adhesive materials commonly used in sanitary dressings, which acts as a contact surface and union with the skin, and a second inner face, on which it is arranged concentrically a second body (3), also of flat surface, circular geometry and diameter smaller than the first body (2).
From the central area of the second face of the second body (3) part a tubular channel (4), with hollow cylindrical geometry and interior similar to a cannula, which projects towards the outside of the device, as shown in Figure 2. Said channel (4) acts as a means of entry of the sheath (1) which, through respective holes made in the second body (3) and in the first body (2) in correspondence with the internal hole of the channel (4), It reaches the skin incision made in the individual to advance to the kidney.
The channel (4) incorporates fixing means (5) of the position of the sheath (1) which, in the case of the described preferred embodiment, comprise a clamp-type perimetral retaining ring, manually operable. Said fixing means (5) immobilize the sheath (1) in a certain position, for example when the area of the
5 kidney where the stones are and begin to be extracted by fragmentation or aspiration, thus preventing the sheath (1) from moving and causing complications such as bleeding. Once it is desired to remove or modify the position of the sheath (1), the fixing means (5) are released manually, and these actions can be performed.
In a first alternative embodiment, shown in Figures 3 and 4, both the first body (2) and the second body (3) have a discontinuity (6) practiced to facilitate the introduction and positioning of the sheath (1) in a oblique position to the skin incision, also allowing the evacuation of the irrigation liquid used
15 during the surgical procedure. Said discontinuity may or may not also occur in the case of the channel (4) and / or the fixing means (5).
A second alternative embodiment, shown in Figure 5, comprises as fixing means (5) villi that, starting from the inner perimeter of the channel (4) 20 converge at its center, leaving a central hole free.

a first laminar body (2) of flat surface, with a face covered with adhesive material to adhere to the skin of the individual, and a central hole, a second body (3), laminar and flat surface, disposed of concentric fan on the first body (2), with a central hole coinciding with the central hole of the first body (2), a tubular channel (4), with a hollow interior, which, starting from the center of the second body (3), projects outwards from the device, with a central hole facing the respective holes of the first body (2) and the second body (3) for entering the sheath (1) through the skin incision, and fixing means (5) for immobilization of the sheath (1) in a certain position within the channel (4).
